Here are links to all of the forms you will need to complete prior to your appointment. 

Please complete the your form on the DAY OF your service, or you will have to re-do it at the shop.

You will need to include a picture of your Government Issued Identification with your consent forms. Please note that acceptable forms of Government Issued Identification include passports, passport cards, tribal IDs, state-issued driver's licenses and IDs, and birth certificates.

If you are a minor getting a piercing, you will need your legal guardian to complete a section of the consent form and include a picture of their Government Issued Identification as well. Please note that acceptable forms of Government Issued Identification include passports, passport cards, tribal IDs, state-issued driver's licenses and IDs, and birth certificates. WE DO NOT ACCEPT SCHOOL IDs AS IDENTIFICATION.
